Email the completed form (together with the supporting documents) to hudsonpark.ps@education.wa.edu.au
Supporting documents:
- Child’s Birth Certificate or Passport
- Current Australian Immunisation Register (AIR) Immunisation History Statement
- Proof of Residential Address (Utility Bill or Rental Agreement)
- Current visa documentation for the child and all parents/carers (if applicable)
